Course Description
This unit describes the skills and knowledge required to organise, provide and monitor support services within the limits established by an individualised plan. The individualised plan refers to the support or service provision plan developed for the individual accessing the service and may have many different names in different organisations.
This unit applies to workers who provide support under direct or indirect supervision in any community services or health context.

Details of Learning Activities
Workshops where information is shared through talks and discussions, and exercises are conducted to apply learning. Time is spent during workshops for students to work on assessment tasks. It is an expectation that students also work on tasks outside of workshop time.
Teaching Schedule
Session 1 Develop relationships with older people Recognising and respecting older people’s social, cultural and spiritual differences Confidentiality and privacy Identify physical and social enablers and disablers impacting on health outcomes and quality of life Session 2 Recognise and support individual differences Consider the person’s individual needs, stage of life, development and strengths when engaging in support activities Work with the person to identify actions and activities that support the individualised plan and promote the person’s independence and rights to make informed decision- making Recognise, respect and accommodate the person’s expressions of identity and sexuality as appropriate in the context of their age or stage of life Session 3 Respect individual differences to ensure maximum dignity and privacy when providing support Deliver services ensuring the rights of the older person are upheld Assist the person to identify opportunities to utilise their strengths, while communicating the importance of using available support when required Encourage the person to adopt a shared responsibility for own support as a means of achieving better health outcomes and quality of life Session 4 Identify and discuss services which empower the older person Monitor own work to ensure the required standard of support is maintained Identify aspects of the individualised plan that might need review and discuss with supervisor Recognise signs consistent with financial, physical or emotional abuse or neglect of the older person and report to an appropriate person Assist the person to access other support services and the complaints mechanisms as required Session 5 Provide support that allows the person to self manage their own service delivery as appropriate Promote and encourage daily living habits that contribute to healthy lifestyle Identify breaches of human rights and respond appropriately Identify hazards and report according to organisation procedures Session 6 Identify and respond to situations of potential or actual risk within scope of own role and report to supervisor as required Complete and maintain documentation according to organisation policy and protocols Discuss situations of risk or potential risk associated with ageing Store information according to organisation policy and protocols Session 7 Practice session pre-placement -1 Session 8 Practice session pre-placement- 2 Session 9 Practice session pre-placement-3 Session 10 Practice session pre-placement-4
