Course Description

This course describes the knowledge and skills required to provide case management to clients who have already been assessed and whose needs have been identified as extending across a number of areas, such as aged care, community care, disability, mental health, drugs, alcohol or homelessness.

  • Complete the AUDIT (Alcohol and Other Drugs
  • Complete an Individual Treatment Plan (ITP)
  • Complete a referral form

  • Group presentations – attend case management meeting

Overview of Assessment

Your assessments will be based around a Case Study "James".  There will be six (6) learning tasks, each of which will be completed after each class for this competency.


Assessment Tasks

Assessment Task  – Individual Treatment Plan (ITP)

1. In week one (1) Students will form case management teams (3-4 students per group)
2. Each case team will be given a case study of a client with a combination of three (3) or four (4) identified needs for treatment depending on group size (case study predefined by teacher, each student group will have the same case study, however for groups of four, one extra issue will be added to ensure all group members have an assigned role to play in the case)
3. The student’s case team will consist of case workers from the respective agencies involved with their client’s case study (e.g. AOD; MH; Housing; Child Protection etc.)
4. Each case team member must research their clients specific issue to gain a better understanding of their client’s specific issues and needs
5. Students will work on their Individual Treatment Plan throughout the first three (3) classes and during class three (3) their Individual Treatment Plans will be submitted for marking
 

Due date 20.4.2015

 Attend a case meeting and report on client progress

1. Following on from AT1 - Each case team member must research their clients specific issue to gain a better understanding of presentation, behaviour, affect and effects
2. Students will work on their progress reports throughout class 4 and during the final class (5) will attend a mock case management meeting (run by the teacher acting as the case manager) to present their client Progress Reports (the reports are designed to follow the development of the ITP’s submitted for AT1)
3. Each student is expected to have built on their background knowledge of the specific issue they have been assigned to work with and demonstrate this through oral discussions of their client’s progress during the meeting
4. The mock case management meetings will run for 15 mins each
5. Students will have their Progress Report marked on the day along with their presentation grade
