Course Description

This unit describes the knoweldge and skills required to facilitate all aspects of case planning.

Develop an appropriate case management plan

Performance Criteria:

1  Develop a case management plan to reflect initial assessment of needs
2  Identify the full range of appropriate immediate, short and long term needs of the client and other relevant parties
3  Develop action plans to reflect:
• integration of expertise of relevant stakeholders and other service deliverers
• negotiated and agreed goals and operational processes
• a range of strategies to address each goal and to maximise participation in plan
• appropriate resource allocation
• agreed responsibility for delivery
• realistic and agreed indicators of success
• rights and responsibilities of client
4  Establish processes for monitoring and changing case plan
5  Identify strategies to deal with complex or high risk situations

1  Facilitate information sharing with the client by establishing an appropriate rapport with the client and implementing appropriate procedures including:

• facilitate discussion
• resolve conflict where relevant
• identify, negotiate and record outcomes
2  Identify and agree boundaries and processes within service delivery, including:
• rights, roles, responsibilities, decision-making processes, accountability and outcomes
• ways of addressing experience, skills, values and development of participant
• impact of statutory mandates on interventions, the client and significant others
• the impact of value systems of worker, client and key stakeholders on outcomes
• information sharing and planning
• appropriate conflict resolution techniques to be employed

4  Provide information on rights of appeal and avenues of complaint so client understands their rights

Manage case work activites and processes.

Performance Criteria:

1  Implement strategies to continually monitor the effectiveness of case management processes against agreed goals, relevant services and programs, client and stakeholder satisfaction
2  Assess the need for changes in case plan, including the need for ongoing intervention, and develop strategies for alternatives as appropriate
3  Successfully negotiate with relevant parties, any proposed changes arising from case review
4  Utilise processes for case closure as appropriate, to comply with organisation procedures

Details of Learning Activities

Classes are scheduled on the last Friday of every month throughout the year.

Students will be given readings in preparation for each workshop.

Led by industry leaders, these workshops take students through a variety of in-class tasks and scenarios with the intention of extending their current working knowledge of Youth Work practice.

The work undertaken in class is invaluable for the assessment tasks.


It is expected that time spent out of the classroom will be dedicated to completing the various assessment tasks listed below.
Students will be expected to make use of the Learning Hub as part of their on-line studies which are part of the course.

Overview of Assessment

Assessment in this unit enables participants to gain skills and knowledge that are directly applicable to their workplace while being encouraged to share current work practices. Assessment tasks involve team and individual projects and written assessment including questions and case studies. Based on your individual learning plan, you will be offered a number of learning strategies including workshop attendance, project work development and workplace support to your assessment needs for this program.


Assessment Tasks

The assessment tasks for this course are:

• UNDERPINNING KNOWLEDGE QUESTION -To research a range of materials and address the set questions by discussing with peers and documenting conclusions drawn. Discussion and reflection to be focused on how Youth Work concepts and principles covered in the readings and workshops apply in the context of developing and supporting a community of practice. 


• EVIDENCE PORTFOLIO A compilation of evidence examples of the students’ roles and responsibilities within the youth work program and of work undertaken within their own community/organization.
