This course describes the knowledge and skills required to deliver targeted specialised client services based on individual case management and intervention. This course may apply in a range of community service contexts.

Overview of Assessment
Assessment for this course is based on completion of three (3) pieces, all of which will be completed in class time. However there are elements of the final assessment task that requires some research by all students. Students will complete each task individually; however students will work in groups to achieve this outcome.
Students will form case management teams in week one (1) and remain in those teams for the duration of this course.

This course has been clustered (grouped) for assessment with the course CHCCS500B Conduct complex assessment and referral. For this course (CHCCW503A Work intensively with clients) you will be required to complete the assessment tasks as described below:
Formative Assessment Tasks (to be completed in class):
Task 1 - this task involves the completion of identification and implementation of case management strategies.
Task 2 - this task involves the monitoring and evaluation of a case study.
Task 3 - this task involves the completion a case closure documentation.
Major Assessment Task:
Case Study: for this task you will be required to choose a case study from your work or workplacement, select a case management model which is most appropriate to your case study and document everything from assessment through to discharge planning.
