ITS service forms

RMIT staff requiring the services of Information Technology Services should make their requests via the following forms.

The majority of forms can be found on the Service and Support Portal. For access to online forms, you will need to log in with your staff ID. If the form required is not an online form, download it and send it via the facsimile number indicated on the form.

AV and event booking

This form must be completed if you are planning an event on one of our Victorian campuses or sites and need to book audio visual (AV) and IT services and equipment.

Call Centre monitoring or recording request form

Please complete this form if you require a phone extension within a Call Centre to be monitored or recorded for customer service training and coaching purposes. You can also use this form to request the removal of monitoring or recording from an extension.

Printing service

Fill out the online printing form if you require a printing device to be installed, moved or disposed of. You can also use this form to request a settings change to an existing device or have additional functions or facilities added.

    • Obtain access for a new user
    • Change access temporarily due to acting in authority during a staff absence
    • Change access for an existing user, due to change of position within RMIT
    • Remove access.

  • Student Lifecycle Systems enhancement request (DOCX 68 KB)
    • Use this form to request an enhancement to any Student Lifecycle System managed by Information Technology Services.

    This form must be completed to:
    • Obtain access for a new user
    • Change access temporarily due to acting in authority during a staff absence
    • Change access for an existing user due to change of role
    • Remove access due to change of role, or departure from the University.

Telephony and voicemail

  • Telephone service request form
    RMIT staff members can apply for any number of telephone services that have been approved by local management. These services include new handsets; telephone extension services, STD/IDD access, voicemail. After submitting the telephone service request, please allow five working days for processing. To create or join a pickup group, please contact the Service and Support Centre and advise of the numbers needing to be in the pickup group.
  • Nortel or Avaya phone return request form
    Please complete this form to request the return of any standard Nortel or Avaya telephone handset that is no longer required.

TRIM (Electronic document and records management system)

  • Staff should complete the TRIM access request form to request access to the University's Electronic Document and Records Management System.

Update contact details

  • Update contact details web form for updating information in staff web profile, Google Contacts and telephone directories.
