Course Description

This unit describes the knowledge and skills required to provide support for people to sustain quality of life according to individualised plans.

Learning Outcomes

The learner will be able to confidently demonstrate their knowledge effectively in the role and responsibilities of self and others in the organisation, the importance and meaning of home and belongings to individuals,  rationale and processes underpinning individualised planning to support client’s strengths, interests, health and emotional well being and self determination, basic principles of person-centred practise, strengths-based practise and active support, duty of care requirements when supporting an individualised plan, contemporary social aspects relating to ageing and disability,  factors contributing to a client’s ability to participate in the community, role of carers, safety and security risks associated with ageing and with disability , strategies for minimising risk , principles and practises for establishing and maintaining appropriate relationships with people required to support an individualised plan. Their knowledge acquired will give the leaner the ability to participate in the  interaction with client assessment process and assessors, to be able to identify resources that will support the individualised plan,  processes and practises that support skill development and/or maintenance,  to identify and participate in  documentation and reporting requirements and procedures while upholding the principles and practises of confidentiality and privacy in all actions of this unit. in addition the leaner will have the ability to follow organisation polices and procedures, apply reading and writing skills required to fulfil work role in a safe manner as specified by the relevant organisation/service. They shall have acquired the skills to communicate effectively and safely within their work role including the ability to follow work related instructions and directions and the ability to seek clarification and comments from supervisors, clients, colleagues and to work effectively  with supervisors, clients and colleagues.


Overview of Assessment

The leaner shall be assessed within the context of this unit by observation in the workplace, and may include written assignments/projects, case studies and scenarios analysis, oral and written questionaires and role play simulation.
