Course Description
This course describes the knowledge and skills required to develop, implement and monitor support requirements for people to sustain quality of life according to individualised plans.

On successful completion of this course, you will have developed and applied the skills and knowledge required to demonstrate your competency in the above elements.
Details of Learning Activities
In this course you will acquire knowledge about the history and context of individualised planning for people with disabilities and the current principles and practices in supporting people with disabilities to develop their own plans. Guest speakers from industry with individualised planning roles will be sought and field visits to services undertaken to hear about the ways in which disability services facilitate individual planning. In group and individual classroom activities you will practise the processes of identifying goals and strategies. The Certificate IV in Disability includes a field placement in a community disability service of 200 hours across the whole program and the knowledge and skills you gain in this course will be developed further during the placement.

During sessions you will be directed to relevant government, non-government and industry sources of information e.g. the Department of Human Services (DHS) website, National Disability Insurance Scheme (NDIS), National Disability Insurance Agency (NDIA), community support organisations with roles and responsibilities in individualised planning. The field placement agency will also have relevant policy and practice documents that will add to your resources.
Overview of Assessment
Assessment is ongoing throughout the course. Assessment will incorporate a range of methods to assess performance and the application of knowledge and skills and will include:
Individual plan preparation, short answer questions and a written report from your field placement.
Assessment Tasks
There are three assessment tasks for this course:
1. Individual Plan preparation: In-class, Group and individual activity - students will work as a group to develop a set of features to be included in an individual support plan. Each student will then develop their own individual support plan based on a goal/s for 2015.
2. Short answer questions: Students answer a set of questions on the theories, definitions and practices in individualised planning.
3. Written Report: from Field Placement, students will write a report based on their observation of and participation in individualised planning and and related support procedures during their field placement.
