Course Description

This course describes the skills and knowledge required to undertake case management meetings to plan, monitor and review service provision.

Workers at this level work autonomously and are responsible for own outputs within organisation guidelines.

This course applies to work in a range of health and community services contexts.

This course is delivered and assessed as a cluster with:

CHCCS004 Assess co-existing needs

Overview of Assessment

This course is assessed in conjunction with CHCCS004 Assess co-existing needs

Assessment task 1: Role-Play

Assessment task 2: Role-play

Assessment task 3: Written Report 

Assessment task 4: Short Answer Questions

You will be given two (2) attempts for each assessment to demonstrate the required knowledge and skills. A third (3rd) attempt can only be offered after discussions between teacher and the Program Coordinator and is not automatic. Resubmissions beyond two (2) attempts are at the discretion of the teacher and/or coordinator.
