Overview of Assessment

This course is assessed with the course CHCCS0015 Provide individualised support.

The assessment tasks are as follows:

Task 1: Case Study - Empowering and supporting a client

Task 2: Portfolio of evidence - Empowering and providing individualised support to clients

Task 3: Direct observation- Providing support to clients

Task 4: Cluster quiz

Assessment Tasks

Task 1: Empowering and supporting a client

For this task, you will identify actions to take to assist the client and to address barriers in achieving their goals. From the perspective of a disability support worker, you are to review a fictional client’s story and support plan.  Using this information and the information gathered from the workshops and learning materials, work through the questions and statements in the Report document. The client's profile and plan are based on several clients of Victorian disability service providers. Your teacher will facilitate a group discussion during a workshop to discuss the client, providing individualised person-centred support and action to empower the client.

Task 2: Empowering and providing individualised support to clients

For this assessment task, you will develop a portfolio of evidence to demonstrate your ability to facilitate the empowerment of people with disability and to provide individualised, rights-based support using a person-centred approach. You will identify three different clients who you are currently or have recently provided support. You will provide evidence of how you have provided support.

Task 3: Direct observation- Providing support to clients

You are to be observed by your assessor in the workplace reviewing the individuals support plan, ensuring that you demonstrate your skills and knowledge of empowerment, supported decision making, and dignity of risk.  The support must be identified in their support plan and be provided by you using a person-centred approach.

Task 4: Quiz

There are 29 knowledge questions that need to be answered satisfactorily to complete this task. The questions are related to the two units of competencies that you are being assessed against.
