Course Description

This course describes the skills and knowledge required to undertake case management meetings to plan, monitor and review service provision.

Workers at this level work autonomously and are responsible for own outputs within organisation guidelines.

This course applies to work in a range of health and community services contexts.

This course will be delivered via distance learning in Semester 2, 2020.

Details of Learning Activities

There will be a range of learning activities to engage students with course material to develop skills, knowledge and understand of case work. The learning activities will be interactive and will include: group work activities, peer learning, discussions, case studies, reflective learning, presentations and practice learning exercises. Note: All learning activities are delivered via remote learning in Semester 2, 2020.

Assessment Tasks

 

Assessment Task 1: This assessment will require students to adopt the role of case manager and complete three role plays and written tasks. Students will assess and identify needs of 3 different clients from the provided case scenario. Student will then complete client assessment forms to document client information and needs gained from the role plays. Student will then be required to reflect on their learning to develop skills and practice improvements in working with clients and assessing needs.

Assessment Task 2: This assessment will require students to complete 3 role plays meetings with their teacher who will play the role of the workplace supervisor and complete written task. Students will prepare and plan for how they will address client needs identified from assessment one. Students will then complete a case plan and referrals for all 3 clients.

Assessment Task 3: This assessment will require students to complete 3 role plays meetings with their teacher who will play the role of the workplace supervisor and complete written task. Students will review all 3 clients progress that they have been working with through assessment task 1 and 2. Students will then complete 1 case review form and 2 closure forms following a comprehensive review of each client from the role play task.

Assessment Task 4: This assessment will require students to complete short answer knowledge questions to demonstrate knowledge, understanding and learning from this unit.
