Course Description

This course describes the skills and knowledge required to contribute to the care of a person with chronic health problems by performing nursing interventions that support the person’s needs and assist them in maintaining an optimal lifestyle.

 

This course applies to enrolled nursing work carried out in consultation and collaboration with registered nurses, and under supervisory arrangements aligned to the Nursing and Midwifery Board of Australia regulatory authority legislative requirements.

 

This course includes a Work Integrated Learning experience in which your knowledge and skills will be applied and assessed in a real or simulated workplace context and where feedback from industry and/or community is integral to your experience.


This course includes clinical placement.

Learning Outcomes


The critical learning for this course takes place in clinical placement within the health sector and within simulations in RMIT laboratories.

You will be instructed in skills through demonstration, modelling and role-play. Skills development is supported by lectures on theory and underpinning knowledge, classroom discussions, group work, research and reflective practice.

Details of Learning Activities

This course describes the skills and knowledge required to work respectfully with people living with chronic health problems. For many Australians measures of health and wellness are not seen as a problem but just something to live with. A retired footballer may view his back problems as a badge of honour. A new mum may find she has stress incontinence that requires pelvic floor exercises and time to overcome. A teenager with Type 1 diabetes may ignore dietary constraints that could lead to adverse complications later in life. 

The Australian Government has released the National Strategic framework for Chronic Conditions 2017 acknowledging that chronic conditions is the preferred term. As a result this course will refer to chronic conditions rather than chronic problems or diseases.

Understanding some chronic health conditions will help the student nurse to undertake nursing work in a vast range of settings, with a wide variety of people living with chronic health problems and conditions. There are far too many diseases and conditions to consider during this course however you will be given the tools to conduct your own learning as you embark on your nursing career. No doubt you will encounter people living with different diseases and conditions that fit the criteria of chronic health problems.

Overview of Assessment

This assessment will incorporate a variety of methods including written, oral, practical tasks and activities.

 

You will be asked to personally demonstrate to your teacher/assessor the practical skills gained during this course to the relevant industry standards. Assessment activities will occur throughout this course and feedback will be provided at regular intervals

Assessment Tasks

There are three key assessments that you will need to successfully complete in order to be assessed as competent in this unit.

HLTENN012 - AT1 of 3: Weekly Quizzes

Students are required to complete weekly quizzes based on the knowledge they have acquired through classes and learning material. Each quiz will be available for one week only. Failure to submit on time, as with any assessment task, will require extension or special consideration as outlined in myRMIT.

CHCDIV002 - AT2: Nursing Care Plan Assessment 

Students are required to analyse health information and the clinical presentation of one person in a simulated environment, Possible nursing interventions related to their chronic health conditions,will be developed into a care plan under exam conditions.

HLTENN012 - AT3 of 3: Clinical Placement 

Students are to demonstrate that they have performed nursing interventions in the workplace to contribute to the support of 2 people who have chronic health problems.

Assessment conditions

Describe how assessments meet the assessment conditions

 

Skills must have been demonstrated in the workplace with the addition of simulations and scenarios where the full range of contexts and situations cannot be provided in the workplace. The following conditions must be met for this unit:

Placement in a suitable location to address all student performance criteria where possible although the patient cohort will determine the scope of actual experiences available. Placement organisation, facilitation and evaluation is conducted by the Placement Coordinator at RMIT.

Additional learning and assessment is supported and conducted in the class room and nursing laboratory.

use of suitable facilities, equipment and resources in accordance with the Australian Nursing and Midwifery Accreditation Council’s Standards including:

Students can understand assessment data including a variety of forms and resources introduced to students during class time. The ability to interpret these forms, set goals and implement and monitor care as described in this competency

  • progressive notes of a de-identified person’s medical history

Simulated patient histories are used in Assessment 2.

Students will have access to patient progress notes and histories. Students are aware of their legal and ethical obligations and the NMBA regulations for Enrolled Nurses and Code of Practice.

Students are issued a Placement log book that includes: Introduction to their placement, the units of competency to be assessed, learning objectives, orientation and expectations.

Interim review is conducted by the Clinical Educator 1 week into the placement

 

Modelling of industry operating conditions including access to real people for simulations and scenarios in enrolled nursing work.

 

 

Assessors must satisfy the Standards for Registered Training Organisations (RTOs) 2015/AQTF mandatory competency requirements for assessors.

In addition, assessors must hold current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia.
