Course Description
This course describes the skills and knowledge required to contribute to the care of a person with chronic health problems by performing nursing interventions that support the person’s needs and assist them in maintaining an optimal lifestyle.
This course applies to enrolled nursing work carried out in consultation and collaboration with registered nurses, and under supervisory arrangements aligned to the Nursing and Midwifery Board of Australia regulatory authority legislative requirements.
This course includes a Work Integrated Learning experience in which your knowledge and skills will be applied and assessed in a real or simulated workplace context and where feedback from industry and/or community is integral to your experience.
This course includes clinical placement.

Learning Outcomes
The critical learning for this course takes place in clinical placement within the health sector and within simulations in RMIT laboratories.
You will be instructed in skills through demonstration, modelling and role-play. Skills development is supported by lectures on theory and underpinning knowledge, classroom discussions, group work, research and reflective practice.
On completion of this course you should be able to:

Details of Learning Activities
This course describes the skills and knowledge required to work respectfully with people living with chronic health problems. For many Australians measures of health and wellness are not seen as a problem but just something to live with. A retired footballer may view his back problems as a badge of honour. A new mum may find she has stress incontinence that requires pelvic floor exercises and time to overcome. A teenager with Type 1 diabetes may ignore dietary constraints that could lead to adverse complications later in life.
The Australian Government has released the National Strategic framework for Chronic Conditions 2017 acknowledging that chronic conditions is the preferred term. As a result this course will refer to chronic conditions rather than chronic problems or diseases.
Understanding some chronic health conditions will help the student nurse to undertake nursing work in a vast range of settings, with a wide variety of people living with chronic health problems and conditions. There are far too many diseases and conditions to consider during this course however you will be given the tools to conduct your own learning as you embark on your nursing career. No doubt you will encounter people living with different diseases and conditions that fit the criteria of chronic health problems.

Overview of Assessment
This assessment will incorporate a variety of methods including written, oral, practical tasks and activities.
You will be asked to personally demonstrate to your teacher/assessor the practical skills gained during this course to the relevant industry standards. Assessment activities will occur throughout this course and feedback will be provided at regular intervals
Students must pass each of the following assessment tasks to demonstrate competent.

Assessment Tasks
There are three key assessments that you will need to successfully complete in order to be assessed as competent in this unit.
HLTENN012 - AT1 of 3: Weekly Quizzes Students are required to complete weekly quizzes based on the knowledge they have acquired through classes and learning material. Each quiz will be available for one week only. Failure to submit on time, as with any assessment task, will require extension or special consideration as outlined in myRMIT. |

CHCDIV002 - AT2: Nursing Care Plan Assessment Students are required to analyse health information and the clinical presentation of one person in a simulated environment, Possible nursing interventions related to their chronic health conditions,will be developed into a care plan under exam conditions. |

HLTENN012 - AT3 of 3: Clinical Placement Students are to demonstrate that they have performed nursing interventions in the workplace to contribute to the support of 2 people who have chronic health problems. |
