Course Description

This unit deals with the knowledge, skills and attitudes required by a dental hygienist to work in orthodontic practice and perform procedures that are prescribed by an orthodontist and comply with national dental hygiene legislated regulations of practice.
Occupational health, safety and welfare practices and infection control procedures and policy are adhered to at all times. All procedures are performed adhering to the principles of informed consent with operator and patient safety and comfort maintained and ensured at all times.

Element:

1. Prepare surgery for orthodontic procedure

Performance Criteria:

1.1 Review patient case notes to confirm instrument, material and equipment selection, orthodontist prescribed treatment and patient individual requirements

1.2 Maintain instruments, materials and equipment according to relevant guidelines for instrument maintenance

2. Prepare patient for the pre-orthodontic treatment phase

Performance Criteria:

2.1 Review patient‟s medical history and records to determine any contraindications to proposed treatment modalities and to confirm treatment prescription

2.2 Inform the patient of the proposed/subsequent procedures and any sensations which may be experienced. Consent is confirmed with the patient and an opportunity to ask questions is provided

2.3 Take a full set of intra oral and extra oral photographs according to relevant guidelines for orthodontic record taking to include digital and film based imaging

2.4 Take orthodontic impressions and bite registration for the construction of diagnostic study models

2.5 Investigate and evaluate the patient‟s current oral health status to provide a customised oral health program to support the prevention of dental disease and trauma with specific emphasis on orthodontic procedures and attachments

2.6 Provide dietary analysis and counselling with particular reference to maintenance and adherence of orthodontic attachments and prevention of dental disease and biofilm accumulation

2.7 Communicate and consult with the supervising orthodontist as required during recording taking procedures

3. Place and remove elastomeric separators

Performance Criteria:

3.1 Review patient case notes to confirm treatment prescription for the placement of separators and adhering to principles of informed consent, explain procedure and provide opportunity for questions to be asked

3.2 Separators are placed as prescribed, with the use of separating pliers or dental floss where applicable, to ensure patient and operator comfort and safety

3.3 Accurately record the number and location of the separators placed in the patient‟s records for future reference

3.4 Communicate effectively with the patient to minimisepatient discomfort

3.5 Provide clear post operative instructions with regard to post treatment pain and discomfort management, oral hygiene care and possible displacement of separators

3.6 Explain the procedure to remove separators and advise of associated sensations

3.7 Note the number and location of separators and cross reference with the patient‟s records

3.8 After a timeframe as recommended by the supervising orthodontist remove separators carefully with the patient‟s comfort in mind

3.9 Perform a thorough prophylaxis following the safe removal of all separators and in preparation for the placement of orthodontic attachments

4. Prepare the patient and place the orthodontic attachments as prescribed by the orthodontist

Performance Criteria:

4.1 Review patient case notes to confirm treatment prescription for the placement of orthodontic attachments and adhering to principles of informed consent, explain procedure and provide opportunity for questions to be asked

4.2 Select and prepare attachments as prescribed by the supervising orthodontist

4.3 Prepare the teeth and isolate as required with the use of retractors, cotton rolls or other appropriate apparatus

4.4 Bond/cement/place orthodontic attachments following manufacturer‟s instructions, procedure, observing tooth position, anatomy and occlusion whilst adhering to the supervising orthodontist‟s treatment prescription

4.5 Remove excess material from around orthodontic attachments and examine surrounding hard and soft oral tissues for residual deposits and remove as required

4.6 Select and place archwire as prescribed by the orthodontist

4.7 Select and fix appropriate module/ligature for archwire fixation

5. Remove and replace archwire as prescribed by the orthodontist

Performance Criteria:

5.1 Examine and prepare patient for archwire removal

5.2 Remove modules, ligatures, or other attachments

5.3 Remove existing archwire/s using appropriate instruments

5.4 Place and fix archwire as prescribed by supervising orthodontist

6. Provide patient education and oral hygiene instruction for the care of the oral tissues and orthodontic appliances

Performance Criteria:

6.1 Discuss dietary advice, oral hygiene care instructions and pain control strategies with the patient and their carer

6.2 Provide instructions for the care and maintenance of orthodontic attachments and if required, elastics, headgear and/or removable appliances are discussed with the patient and their carer

7. Prepare the patient and remove orthodontic attachments as prescribed by the orthodontist

Performance Criteria:

7.1 Provide clear patent instruction regarding all procedures involved with the removal of orthodontic attachments

7.2 Assess dentition with regard to current and previous dental condition(s) prior to the removal of the orthodontic attachments

7.3 Observe patient comfort during the procedure and modify treatment as required to alleviate pain and/or discomfort

7.4 Remove orthodontic attachments according to manufacturer‟s recommendations, ensuring the protection of tooth structure at all times

7.5 Remove residual cement, resin and calculus with a power scaler, hand instruments or hand-piece to achieve a clean tooth/root surface

7.6 Take impressions for the construction of retainers/study models

7.7 Take a set of post treatment extra oral and intra oral photographs as required

7.8 Perform a thorough prophylaxis and apply fluoride as required

8. Place fixed retainers

Performance Criteria:

8.1 Confirm orthodontist‟s treatment prescription and refer to the patient records and treatment plan

8.2 Inform the patient of all the necessary procedures and sensations associated with the application of a bonded retainer. Informed consent is given by the patient

8.3 Remove deposits from the teeth, prepare tooth surfaces for the application of the retainer following guidelines, procedure and manufacturer‟s instruction 8.3 Place and fit the retainer wire in the appropriate position using the appropriate/prescribed fixing agent to the recommendations and manufacturer‟s instruction

8.4 Check all tooth/root/composite resin surfaces are smooth

8.5 Check occlusion to maintain occlusal harmony and to ensure patient comfort and safety. Eliminate any occlusal interferences as determined

8.6 Provide post operative oral health care instruction and arrange for maintenance review appointment(s)

Assessment Tasks

 FORMATIVE TASK 1:  Create a pamphlet

“Caring for your teeth and gums becomes more difficult once you have braces.”

Create a pamphlet for your patient who has the following orthodontic appliances:

  1. Clear plastic retainer
  2. Hawley retainer/removable appliance
  3. Fixed lingual retainer

The pamphlet should contain information on oral hygiene recommendations; nutritional counselling; chemotherapeutic agents as well as information on what do in an emergency.

Please include interesting pictures/photos in your pamphlet.  The pamphlet should be easy for patient to understand.

FORMATIVE TASK 2:  Visit to an orthodontic practice

As part of the practical component of this unit, observation in an orthodontic practice is mandatory.  You are required to attend:

WRITTEN REPORT OF OBSERVATIONS

You will be required to record the hours spent at the practice, as well as all the procedures observed.  See #3 to #9 above for procedures list and activities list for details.

On completion of your observation period, prepare a written report explaining the procedures and practices observed, as well as materials and equipment used for each procedure.

Submit the record of attendance and written report with a completed coversheet by the due date.

Late work that is submitted without an application for an extension (see below) will not be corrected. APPLICATION FOR EXTENSION OF TIME FOR SUBMISSION OF ASSESSABLE WORK - A student may apply for an extension of up to 7 days from the original due date. They must lodge the application form (available www1.rmit.edu.au/students/assessment/extension) at least 24 hours before the due date. For missed assessments such as exams- you (& your doctor if you are sick) must fill out a special consideration form. Assignments will be marked Satisfactory/Not satisfactory. Students may be given the opportunity to resubmit the assignment if this is requested by the teacher and approved by the co-ordinator.
