Course Description

In this course you will explore contemporary nursing practice for people experiencing chronic conditions and disability. You will explore the impact of chronic conditions and disability on quality of life from all age groups and cultural backgrounds. You will apply the social determinants of health in the context of community care and be introduced to key concepts in chronic and preventative health interventions. You will review the provision of palliative and end-of-life care using evidence-based guidelines and frameworks for all age groups.

Upon successful completion of this course, you will be able to:

  1. Discuss models of primary and community nursing practice, including chronic conditions management, preventive care and health promotion.
  2. Understand basic epidemiological concepts used for health prevention and promotion in addressing the needs of individuals and communities with speciality health variations .
  3. Analyse the impact of disability, determinants of health and nursing care of common chronic conditions within a public health context.
  4. Examine and apply best-practice principles and evidence-based frameworks in palliative and end-of-life care for all age groups.

This course will use a range of blended learning activities including lectures, lectorials. Throughout the course, you will be able to work both independently and in groups to solve problems using aspects covered in the course.
