Course Description

This course describes the skills and knowledge required to develop, implement and monitor service responses with a person with disability. Work is undertaken within a legislative and ethical framework to ensure the provision of high quality, person-centred service delivery which supports the person’s aspirations, needs, rights and interests.

This course applies to workers in varied disability service contexts. Work performed requires a range of well developed, person-centred skills where some discretion and judgement is required and workers will take responsibility for their own outputs.

Performance Criteria:

1.1 Collaborate with the person with disability and/or their family and/or carer and/or relevant others in actively developing responses that meets the individual’s aspirations, needs, rights and preferences

1.2 Put in place mechanisms to ensure the person’s support information is accurately recorded, maintained and applied to future support activities and responses

1.3 Develop the required responses that cater to individual differences, rights, needs and preferences

Details of Learning Activities

Classes where information is shared through talks and group discussions, and exercises are conducted to apply learning. Time in class, will also be spent working on applied assessment tasks.

Session 1 

Empowerment, individualised person centred planning and supports and community inclusion

  • Over the past thirty years the disability service sector has shifted from a pre-dominantly centre/facility based focus to community based individual person centred planning. Empowerment, individualized services, community inclusion and participation are linked and considered today.

Session 2 

Community participation and inclusion practice

  • Services, issues and solutions to support community inclusion and participation will feature in this workshop. Advocacy and self advocacy are valuable  tools in ensuring that the people we support take their place in their communities and society.

Session 3  & 4 

Complex needs, complex lives and holistic integrated services

  • As part of looking at our clients as individuals we recognise the significance of their whole life and the complex needs they may have. That may include chronic and/or episodic health or mental health conditions, housing, finances, social and community interactions, justice, or employment.

Overview of Assessment

This course is assessed with the course CHCDIS008 Facilitate community participation and social inclusion.

The assessment tasks are as follows: 

Task 1: Facilitating participation and inclusion – Practical task - Simulation

Task 2: Facilitating participation and inclusion – Supervisors report and student work log

Task 3: Facilitating participation and inclusion – practical task- observation

Task 4: Unit Knowledge Questions

 


Assessment Tasks

Assessment task 1 - The purpose of this assessment task is for you to demonstrate your skills in developing, implementing and monitoring a

service response with a person with disability, in a simulated environment.

You are to engage in two (2) simulations with the assessor demonstrating your ability to respond to both simulation briefs and to develop and implement a person-centred service response responses to meet the aspirations, needs, rights and preferences for two (2) people with a disability. 

You will then review and monitor these two individual service responses and make changes that are necessary to improve quality service delivery and:

  • meet the changing needs of the person
  • address a barrier
  • respond to legislative changes

 

Assessment task 2 - To be marked competent in this unit the student is required to obtain a supervisor sign off on their work and to

confirm that they have completed a number of tasks during a minimum of 120hrs of direct support work

As a student you must

  • Acts in a professional and courteous way and respects the rights of other people in the workplace
  • Follows the policies and procedures of the workplace
  • Keeps information about the organisation confidential
  • Pro-actively seeks to develop skills and knowledge while undertaking this assessment task and
  • Complete documentation required by RMIT

Assessment task 3 - The purpose of this assessment task is for you to demonstrate your skills in facilitating your clients’ community participation and social inclusion.

You will be observed by your assessor in the workplace assisting three clients to determine and prepare to access a community opportunity or activity.

You are to identify three clients who will provide consent in being observed by an assessor when working with you to facilitate participation and inclusion.

Following each observation, you will also provide verbal responses to questions related to providing support to your client to help assess your knowledge and how you apply it to different work situations.

you  also must include the following supporting evidence with your assessment submission:

  1. Logbook - re-submit your Task 2 120 hours Logbook
  2. Risk Assessment – to be distributed to students by class teacher

Assessment task 4 -

This assessment is a short-answer knowledge task designed to assess the knowledge required to develop, implement and monitor person centred service responses for community involvement and for the improvement of the lifestyle of a person with a disability.

You will complete this knowledge assessment under supervision in class.

All knowledge assessment questions must be answered correctly, for you to be assessed as satisfactory for this assessment task.
