This unit describes the skills and knowledge required to determine and address specific issues faced by people who have a chronic disease, in the context of contributing to a holistic approach to support.

Welcome to the unit Address the needs of people with chronic disease!

Course description

This unit describes the skills and knowledge required to determine and address specific issues faced by people who have a chronic disease, in the context of contributing to a holistic approach to support.

The unit applies to workers providing services to people with chronic disease in diverse community services and health contexts.

How this course runs

This unit is run over ten (10) weeks, via face to face teaching and learning.

Learning is contextualised through in-class assessment role plays, in-class presentations, self-directed internet research of selected chronic conditions and or pathologies and work integrated learning (WIL) Student Clinic practical application that demonstrates understanding and management of chronic disease and lifestyle presentation. 

RMIT has various study spaces and computers available to students (booking required Links to an external site.). Building 51, level 2 has open drop-in cubicles available on a first in scenario. RMIT free WiFi data and connection is also available when on site.  

Canvas Learning Management System (LMS Links to an external site.): is the online system for all course and unit and assessment communication. This is also the space where assessments are to be uploaded. 

Assessments

Performance Assessments: Assessment to demonstrate knowledge and application into a practical presentation will be performed In Student clinic,  via treatment of three (3) clients with chronic disease and lifestyle presentation. 

Knowledge Assessments:  Through knowledge questions, in-class role plays and a selected chronic disease presentation. These assessments will include: 

  • required evidence demonstrating knowledge of broad context for chronic disease,
  • World Health Organisation and other models of chronic disease management,
  • continuum of chronic disease,
  • life variables that have an impact on health and well being,
  • impacts of chronic disease,
  • strategies to address and manage chronic disease in the relevant work context,
  • ways to communicate appropriately and effectively with a person with a chronic disease and roles and responsibilities of different people in chronic disease management and their contribution to coordinated service delivery.

Important information regarding Assessment Tasks:

 

Resubmissions are not automatic and will only be approved at the discretion of the Program Coordinator, after consultation with the teacher and each individual assessment may only be resubmitted once. 

All students will be contacted with 7 days by their course coordinator in regards to  resubmission completion dates.

   

Please note an opportunity to repeat clinical placement is not guaranteed and will be at the discretion of the Clinical Coordinator/Program Manager.

 

EXTENSION OR SPECIAL CONSIDERATION

 

If due to illness or other unforeseen circumstance, you are unable to attend on the due date of an assessment (e.g. test, examination, oral, etc.) OR you do attend, but your performance has been affected, you may submit an APPLICATION FOR SPECIAL CONSIDERATION within 5 working days of the due date. If Special Consideration is granted, you will be given the opportunity to complete/repeat the assessment task at a later date. If due to illness or other unforeseen circumstance, you are unable to submit by the due date (e.g. assignment, report, project etc.):
