Course Description

In this unit you will develop the skills and knowledge required to assess capacity to support people with co-existing mental health and alcohol and other drugs issues and to work collaboratively with the person, family and carers in developing a recovery plan.

This unit applies to work with people with co-existing mental health and alcohol and other drugs (AOD) in a range of community services work contexts.

This course is delivered and assessed in a cluster with;

1. Involve the person, carers and families in planning support during recovery

Performance Criteria:

1.1 Identify benefits and issues related to family and carer participation in recovery processes

1.2 Discuss with the person appropriate involvement of family and carers

1.3 Work with the person to identify and agree on specific roles and responsibilities and accurately record in the person’s recovery plan

1.4 Consult with identified family, carers and other relevant stakeholders in line with the person’s consent

1.5 Gauge level of relevant knowledge, skill and involvement of carers and family as a basis for identifying additional support requirements

1.6 Provide information, resources, referrals and education to meet the needs of carers and families within scope of own work role

Week 2

  • Exploring the benefits and risks of collaborative practice
  • Learning about case conferences
  • Considering how members of the multidisciplinary team work together
  • Mapping collaborative relationships

Week 3

We will be looking at the ways in which AOD workers work together to maximize client outcomes and efficiencies.

  • The benefits and disadvantages of specific collaborative practices
  • Introduced to real life examples of collaborative practice

 

Week 4

We will be looking at the ways in which AOD workers work together to maximize client outcomes and efficiencies.

  • Undertaking a SWOT analysis
  • Thinking about collaboration opportunities
  • Working on Assessment Task 2

 

Week 5

We will be learning about case coordination meetings and the inclusion of family members and the client themselves.

We will be;

  • Revisiting the case study about Sarah
  • Undertaking a case coordination meeting
  • Developing a joint support plan with the client, family and another agency / worker
  • Sharing information in networks
  • Representing your organisation
  • Revisiting the Victorian AOD self completion form
  • Working on Assessment Task 3

 

Week 6

 Review

Week 7

We will be;

  • Giving overall feedback on assessments
  • Answering questions / queries
  • Going over topics where knowledge gaps have been identified
  • Sharing expertise with one another
  • Catching up on assessments and class work
  • Working in groups to research the questions in Assessment Task 3

 

Week 8

Today we will be introducing a new case study that requires collaborative practice. We will be looking at a youth specific assessment tool. 

We will be; 

  • Making sure the client themselves feels that they are a crucial part of their own team
  • Considering how to do this when the client is a young person
  • Exploring principles of youth assessment
  • Considering psycho social needs of a client
  • Making referrals to other services
  • Undertaking a crisis assessment and safety planning tool designed for young people

 

Week 9

Today we will be putting our own personal spin on standardised assessment. Bringing values of client and family participation into the assessment experience.

We will be;

  • Making tweaks to the way we assess so that it is more family inclusive
  • Opportunity to work on assessments during class

 

Week 10

Today we will be revisiting case conferences and complex care coordination. We will be considering who should be present at these meetings and how to make it a useful experience.

We will be;

  • Exploring our final case study for this unit!
  • Revisiting all the various members of the multidisciplinary team
  • Considering how the multi disciplinary team can work together in support of a client
  • Tips and tricks for attending a multidisciplinary team meeting

 

Week 11

We will be reflecting on difficult interpersonal dynamics in collaborative work and considering what we bring to the collaborative relationship. We will be working with our final case study for this cluster.

We will be;

  • Working with ‘difficult people’
  • Reframing the notion of ‘difficult people’
  • Reflecting on our past experiences of ‘difficult encounters’ with others
  • Considering the impact of the way we are on the group dynamic
  • Brainstorming positive and constructive responses to common interpersonal dynamics in multidisciplinary teams

 

Week 12

 Review of an ITP involving the client and a multidisciplinary team

Week 13

We will be understanding how to review and evaluate individual treatment plans.

We will be;

  • Reviewing Hudson's ITP
  • Learning about professional networks
  • Researching professional networks that are relevant to our practice
  • Referring to our huge list of members of the multidisciplinary team for inspiration

 

Week 14

Week 15

This is our second last session for this cluster. It is an opportunity to complete any outstanding assessments, seek feedback from your teacher and gather resources you would like  to take forward into your practice.

We will be;

  • Completing all assessments for this cluster
  • Receiving Feedback on assessments submitted so far
  • Opportunity for class discussion and deepening knowledge in areas of interest

 

Week 16

This is our final session for this cluster. It is an opportunity to complete any outstanding assessments, seek feedback from your teacher and gather resources you would like to take forward into your practice.
