Course Description

This unit describes the skills and knowledge required to determine and address specific issues faced by people who have a chronic disease, in the context of contributing to a holistic approach to support.

 

The unit applies to workers providing services to people with chronic disease in diverse community services and health contexts.

1.1 Identify the person's chronic disease conditions and seek information about its possible impacts on health, well being and ability to achieve maximum performance in everyday situations 1.2 Determine current treatments, and other inputs and supports by seeking information from the person or other health professionals 1.3 Determine the level and type of support and assistance required by the person within scope of own role 1.4 Adjust services and support to meet the specific nature of the chronic disease 1.5 Actively involve the person in the development of strategies to self manage their chronic disease

Overview of Assessment

Assessment Tasks

Assessment  1- Short Answer questions in relation to all of the course content taught - Due : 28th August 2024

Assessment 2 - Practical case study assessed within class and a power point presentation on the chronic you are treating - Due : 24th Sept. 2024

Resubmissions-

For each assessment you will be given up to two (2) attempts, however, to be granted the second attempt at any assessment, your first attempt must be an appropriate attempt to try and gain a satisfactory outcome. For example, you must appropriately attempt each question or aspect of the assessment task.If your first attempt fails to meet the above standard, you may not be eligible for a second attempt at that task.A third (3rd) attempt can only be offered after discussions between teacher and the Program Coordinator and is not automatic  

USE OF AI TOOLS IN THE ASSESSMENTS-

AI tools cannot be used in the completion of any of the above assessment tasks. 

In these assessment tasks, you must not use any AI tools (excluding text editing software e.g., Grammarly) to generate any materials, content or ideas related to the task.

Attendance: 

Your learning experience will involve class-based teaching, discussion, demonstration and practical activities.  

It is strongly advised that you attend all timetabled sessions. This will allow you to engage in the required learning activities, ensuring you the maximum opportunity to complete the placement and this course successfully.

Important information regarding Assessment Tasks:

Each individual assessment may only be resubmitted once.

Resubmissions are not automatic and will only be approved at the discretion of the Program Coordinator, after consultation with the teacher.

All students will be contacted with 7 days by their course coordinator in regard to resubmission completion dates.

Please note an opportunity to repeat clinical placement is not guaranteed and will be at the discretion of the Clinical Coordinator/Program Manager.

EXTENSION OR SPECIAL CONSIDERATION

If due to illness or other unforeseen circumstance, you are unable to attend on the due date of an assessment (e.g., test, examination, oral, etc.) OR you do attend, but your performance has been affected, you may submit an APPLICATION FOR SPECIAL CONSIDERATION within 5 working days of the due date. If Special Consideration is granted, you will be given the opportunity to complete/repeat the assessment task at a later date. If due to illness or other unforeseen circumstance, you are unable to submit by the due date (e.g., assignment, report, project etc.) you may submit an APPLICATION FOR EXTENSION OF TIME FOR SUBMISSION OF ASSESSABLE WORK for an extension of time of up to seven (7) calendar days or less.
