This course describes the skills and knowledge required to assess the diverse and multi-faceted needs of people and determine both internal and external services required to meet those needs.
This course applies in a range of community service contexts.
This course is delivered and assessed as a cluster with:
CHCCSM005 Develop, facilitate and review all aspects of case management

Overview of Assessment
This course is assessed in conjunction with CHCCSM005 Develop, facilitate and review all aspects of case management
Assessment task 1: Role-Play
Assessment task 2: Role-play
Assessment task 3: Written Report
Assessment task 4: Short Answer Questions
You will be given two (2) attempts for each assessment to demonstrate the required knowledge and skills. A third (3rd) attempt can only be offered after discussions between teacher and the Program Coordinator and is not automatic. Resubmissions beyond two (2) attempts are at the discretion of the teacher and/or coordinator.
